Individual Organizer Checklist

Did you:

Personal Information:

  • Did you receive an IOU from California last year?
  • Change your address from last year
  • Did you move? Was it for employment or business reasons? Was it to/from another state?
  • Do you wish to have your tax return mailed to an address other than that on the return?
  • Change your marital status
  • Become legally blind or disabled
  • Buy, sell, or short sale any property? Please bring in your escrow statement (HUD-1) and Forms 592-B, 593-B, or 3528A (first home credit)
  • Have any debt relief, foreclosure, or debt forgiveness? Please bring in 1099-A and/or 1099-C
  • Rent for at least 6 months

Dependent Information:

  • Have any changes to dependent information? Bring in the Social Security Card for new dependents
  • Did you adopt a child
  • Pay for child care? Please provide amount paid for each child, as well as the name, address, phone number, and identifying number (SSN or EIN) of care provider
  • Can someone else claim you as a dependent

Education Information:

  • Did you or your dependent attend any post secondary education classes?
  • Use your 529 plan for education expenses
  • Pay student loan interest

Income Information:

  • Make or receive alimony payments
  • Payments on prior year sales of property
  • Trade a vehicle in on the "Cash for Clunkers" program
  • Rental or royalty income
  • Tax exempt interest, disability payments, or other non-taxable income
  • Foreign taxable income or foreign tax
  • Are you a grantor or transferor for a foreign trust? Do you have an interest in, or other authority over a bank account, securities account, or other financial account in a foreign country?
  • Buy or sell securities? Please bring in your statements
  • Receive stock options or grants from your employer, or exercise stock options
  • Contribute to, withdraw from, or rollover a Keogh, IRA, or Roth IRA.  Are you over 70 ½?
  • Unemployment, social security, or railroad retirement benefits
  • Any type of prize, award, loan settlement, tips, gambling winnings, or jury duty fees

Purchase, Sales, and Business Information:

  • Increase employees from 2008? Please bring in payroll records for 2008 and 2009
  • If you are claiming deductions for travel, entertainment, business gifts you must substantiate by adequate records, including: (1) Amount, (2) Time and place, (3) Date and description of gift, (4) Business relationship, and (5) Business purpose.
  • Use your car for business? You must maintain a mileage log!
  • Refinance your home loan? If yes, be sure to bring in your final closing settlement statement.
  • Use a portion of your home as an office
  • Start a new business
  • Sell or dissolve a business
  • Acquire new or additional interest in a partnership, LLC, or S-Corp
  • Purchase assets for use in a business
  • Work out of town at any time
  • Engage in any bartering transactions
  • Job seeking expenses

Miscellaneous Information:

  • Consider making a Roth IRA conversion in 2010
  • Suffer a major casualty such as theft or storm damage
  • Contributions to or distributions from a Health Savings Account (HSA)
  • Receive any COBRA insurance
  • Purchase gasoline, fuel, etc. for a non-highway vehicle?
  • Purchase any items online, by phone, by mail, or out of state, where no California sales tax was charged on it
  • Purchase a Hybrid vehicle
  • Purchase a vehicle? Please bring in your Purchase Agreement Contract
  • DMV fees paid
  • Union dues paid
  • Did you give any gifts of more than $13,000 to any individual?
  • Do you wish to make voluntary California contributions to certain charities?
  • Do you want to allocate $3 to the Presidential Election Campaign Fund?
  • May the IRS discuss your return with your tax preparer?
  • Charitable contributions now need proof of donation.  be sure to keep your receipts
